Mystery Form IMM5540

Hi there. I have a really sort of embarrassing question.

I'm just doing the final check of my application for sponsorship under the common law/family class--nearly done, hooray!! Just one problem, I have a spare form: IMM5540.

It LOOKS like it belongs with my package (the title is Sponsor Questionnaire: Sponsorship of a Spouse, Common-Law Partner, or Conjugal Partner), but it doesn't appear anywhere on IMM5443 (Document Checklist for Spouse or Common Law Partner in Canada). Help! How did I end up filling out this form, and where does it go?

UPDATE: I found out I got the form from here: http://www.cic.gc.ca/english/pdf/kits/guides/3910e.pdf

My question is, did anybody find that this list above was completely different to what the IMM5443 checklist is asking for? It's like two different applications/packages, asking for four photos instead of two (and SO badly worded, whoever wrote these forms sucks at their job).

I'm 100% you need to include that in your application.
Sponsors have 3 forms to fill out and sign:
-Undertaking
-Sponsor Questionnaire
-Sponsor Evaluation

This checklist is for the sponsored person. This is a specific checklist depends on which country you're from. The master checklist IMM5443 is for the sponsor, to check if all the documents (sponsor and sponsored person's) are in before finally submitting it to CPC-M.

You have 2 bunches of documents:
- Sponsor's applications with supporting documentation
- Applicant's applications with supporting documentation

IMM5540 is a mandatory questionnaire for sponsor. IMM5443 is a checklist for applicant to see whether he/she included all applications and supporting documents. Sponsor's documentations are not reflected on applicant's checklist, likewise applicant's documentation are not reflected on sponsor's checklist.

OK, thanks. So it's like a checklist for if we are apart, I get it.

Right now we're living together, and I have done all the medical exams and everything here in Canada. That ate up some photos, so if IMM5443 only requests two photos, should I heed this paragraph from the country specific instructions that says this?


PHOTOS
Supply five (5) recent photos for each member of your family and yourself. Follow the instructions in your guide (see Photos in section on completing the Application for Permanent Residence in Canada) and in Appendix B, Photo Specifications.

If IMM5443 is for the sponsor, howcome it doesn't ask anything about the Sponsor Questionnaire? Thanks for your help, by the way.

You need to submit 5 photos.

I did not use IM5443, I only used IMM5491 as the master checklist.

I think we both confused ourselves. I just realized you were applying inland, and I've applied outland. Therefore, we had different checklists. You're right, you do not have to submit IMM5540 because it is for outland application. However, the counterpart for inland would be IMM5285.

Your master checklist is IMM5443. (inland)
Outland applications' checklist is IMM5491.

Aha, you are amazing. Thanks. And good thinking! I guess that leads me to my final question--definitely submit more photos than IMM5443 asks for? My feeling is that the extra photos are for the medical reports that you do outside Canada, which are then attached to the photos...?

I thought it was asking for 5 photos right? That's what it says in the checklist. Doesn't hurt if you submit a bit more.

These are for your medicals and PR card.
