MPNP - Input Multiple Institutions for Secondary Education

Hi,

I studied in 4 different schools during my secondary educations. This is a print of the online form:

Should I put only the last school (the one that I received a degree) or should I put a separated entry for each institution?

Also, what should I put under the "Program name" field?
