Mother applying with minor - different name in birth certificate (maiden). Need an advice

Hi there,

I am applying with my kid. I am attaching kid's birth certificate as requested. Although it has my maidan name which differs from my current last name.
Do I need to attach my marriage certificate as a proof that we are related and proof of my changed last name?

I couldn't be the one with this question, but wasn't able to find any relevant info.
