Most recent imm5710 forms??? Help

Question... What is the most recent imm 5710 form for the spousal owp? On the cic page beside the form PDF it says October 2015 but when I opened the form it's 04-2015. Is April 2015 correct?

Yikes - I just sent our application last week (Nov 4) and checked all the form dates right before mailing. It said April 2015 at the time. Does this mean we will have to send another form? How will this impact my husband's implied status? Worried!

Really hope so! Though the form is still the April 2015... can't send in an updated one even if I tried.

FYI... it was changed back to April 2015
