MISSING SURNAME ON PASSPORT

Hello All,

I want to apply for FSW 2014 with my wife.
I'll be the primary applicant. I have some queries as below:

1. In my passport my full name is under Given Name field only and surname is blank. But in forms surname is mandatory. How can I complete the form?
2. I already applied for credential evaluation through WES. Is it required for my wife also?

No need to worry , that will be fine . I dont have surname and wife even canadian they stamp my visa
