On form 'Additional Family Informatio', Section A: spouse or common-law partner...
should she list her ex-husband?
Does her 2 year old daughter need a passport? Or just a birth certificate?
On form IMM5490,question 8, 'do you have any other relatives living in Canada?', should she include 2nd cousins?
On form IMM5490, Interpreter Declaration, This form will be interpreted by my friend who speaks fluent Cambodian/English. Is this sufficient? Or must it be interpreted by a notarized interpreter?

1. No, the ex-husband shouldn't be listed in additional familiy information.
2. Birth certificate's fine.
3. No, a 2nd cousin doesn't qualify.
4. Interpreter declaration - don't complete that section unless asked to during the interview.

If the 2 year old is emigrating, she will need a passport.
