Minor Personal Identification

Hi,

I am applying for citizenship for my wife and son (both permanent residents) who have met residency obligations, in one application. I have 2 questions listed below. If you are able to review, and advise, I would truly appreciate it -

  1. Since my wife (one parent) is applying together with my minor son, we are using the "sub section 5(2)" documents for minors for my son. (Note: I have applied for my citizenship last September and am still awaiting a test date; so not a citizen yet myself). Does this sound right?
  2. The Document checklist for minors (sub section 5(2)) lists photocopy of 2 pieces of minor's personal identification, with only one having the minor's photo on it. We were considering including copies of - a) PR card (which has his photo), and b) his Ontario health card (which doesn't have his photo). Would these be acceptable?

1. Yes, your wife can apply with your son under 5(2), one of parent applying with the minor is enough.
2. A PR card cannot be use as a proof of ID for minor application. You can still use a valid copy of his passport and a copy of his health card and yes you can use his Ontario health card as a second ID.

Photocopy of two (2) pieces of personal identification.
Both pieces of identification should show the minor’s name and date of birth, one of which must have a photo on it.
Examples include:
school record;
health insurance card;
a passport page containing their photo and personal details;
a hospital or immunization record, may be provided.
Note: If there is information on both sides of the identification document, photocopy both sides.

Source? Correct me if I’m wrong but it does not say anywhere that a PR card is not a valid form of identification for a minor application.

Hi seniors, I am having problem in filling the form of minor. On 12 - signature when I put the date. Error message is coming. It says that the correct format should be equal to today's date. How do I ensure that my son's date on the form is the same as my application.
Anyone else faced this? Thanks
