IRCC asked my friend to go do his medicals. He went to do it and they updated his portal that he passed after some days. yesterday they updated his portal that he has been refused. What could be the reason? I didnt know it was possible to get refused even after being asked to do medicals.

You haven't said what you applied for so hard to comment. But yes, it's certainly possible to be refused after medical. Being asked to do the medical doesn't mean the application has been approved.
