Hi all,

I am planning to call the doctor tomorrow to make an appointment for the medical examination.

I do not have a IMM 1017 B form? Will the doctor give me this form?

What information should I give to the doctor apart from passport and photo? Do I have to give my Application number to them? Is this application number the one on our Express entry profile page?

tell him that it is an upfront examination. they will generate the number for you. make sure you go with the doctors in panel. it is much easier doing that.
