Medical Test - No form needed to pass the medical test please ?

Hi Folks,

I have the document check in front of me (I got the ITA), for the medical test, CIC did not send me any specific form to give it to doctor prior to the test, is it normal ?

Because when I called the doctor to get an appointment, he asked for a form that I should get before passing the exam, I am confused.

PS: This is what I have in the document check list:

You require a medical exam.

In order to submit your online application, you will need to upload either the information printout sheet, or the IMM 1017B Upfront Medical Report form. Your doctor will give you one of these documents when you complete your medical exam.

Just take the ITA letter...That should be enough...Tell the doctor that this is for EE

Ok, but I am getting more confused.
Why on your profile, does it say: Medical request after you applied (after getting your ITA), it means like your applied without medical test ?

Also, for police check, why on your profile and on other ppl profile, does it say, PC requested way after they apply ? don't we have to upload that all before applying ?

Finally please, if you can clarify, I have the appointment tomorrow with the doc, the initial form he will give me, could I upload it and submit the application, or I can submit until I get the final results ?
