It will be given to you by the doctor/health centre assessing your medical. If the facility is e-medical enabled, then you will get "E-medical Information sheet".

Only problem is that in my case, its written as follows -

Exam Status
501 Medical Examination Required
502 Chest X-ray Examination Required
707 HIV test Referred
712 Syphilis Test (VDRL or RPR) Referred


Can I upload the same certificate?

yes, those status will be updated on their side electronically. Your part is just to upload the paper, with very clear bar code.

712 Syphilis Test (VDRL or RPR) Referred

If you read carefully its writte on the same report that status will only be changed once results have been submitted by the panel physician good luck
