Medical Request form Address

Hi I want to ask if this is normal, my address here in canada is in Saskatchewan my family is outside Canada. On my medical request form IMM1017 my address is correct but my family outside canada stated that their address also is here in Saskatchewan. Is it normal? I'm sure that I stated their address as outside canada

Hi! I just saw your note. I believe you finished all the process.

I have the similar situation.. I am in Canada and my spouse is in India. We got the medical request.. On my form, they have mentioned my current address (Toronto) and in her form they mentioned my address as well, not her current Indian address. What did you do in your case? did you ask them a new form for your family or they just proceeded with the same form?
