Medical report

I gave my medical a week back after getting instructions through email. They had attached IMM 1017E in the email which was filled up and signed by the panel physician. I am not sure if i have to submit that form anywhere or not. Any help would be much appreciated.

Talk to the clinic, they should be submitting your tests electronically.
