Hello again, everyone.

I am a Canadian-born citizen living in the US with my wife and our dauhghter. We are trying to get everything together to apply to come to Canada. My daughter has her Certificate of Citizenship by descent, and obviously I am a citizen, so the only thing we need to do is get my wife her PR card. We will be applying from here in the US (NY state).

I am going through the document checklist for sponsors and sponsored persons and have a question.

I was under the impression that the medical should be provided after the sponsorship application has been accepted, and while the PR is being processed. In other words, we thought the CIC would request the medical once they needed it. From what I can tell, however, it seems like we're supposed to include it in the initial application submission.

Can anyone give me simple instructions on what exactly we're supposed to do and when, insofar as the medical exam (IMM1017) is concerned?

I'm not sure if I understand what you want with instructions.

The medical exam must be done up front with the initial application, like you realized. When you do the exam, the designated practitioner will give you a special receipt, which is the Imm1017. You must submit this receipt with the initial application. That's it. With this receipt, the visa officers will be able to access the medical results online. Make a photocopy of this form for your own records.

I assume you are planning to apply as outland, since you are all living in the US at the moment. With outland applications, it is strongly recommended that your wife take an up front medical exam (available only for the Family Class immigration program) and include it in the application before you send it off. There have been cases where the application was returned for lack of medical exam proof or most commonly, they ask you for the medical before they start processing step 1 of the application (Sponsor approval), which could delay things by a couple of months.

Go here: http://www.cic.gc.ca/pp-md/pp-list.aspx
You can find a CIC panel approved physician in your area. Call them and book an appointment, making sure to emphasize that you want an up front medical exam. After the exam, they will provide you a paper called eMedical , which you will include in your application. There is no need for any form from CIC before doing the medical exam.
