My minor son visited Canada in 2018 on his visitor visa, and in coming September he will return with me(study permit holder) on his older visitor visa. Would he need to renew his medical exam, even if his counterfoil is Valid until October 2022, or will it not affect him as a returning visitor?

No he doesn’t need a new medical.

That's great.
And, as the child of international student, will he require study permit to continue his education in Canada within Nova Scotia particularly, or will his current tourist visa suffice?

Visitor visa should be fine. Would ask for the expiry date for his stay to coincide with the expiry of your study permit so you don't have to apply to extend every 6 months

Will the CBSA at POE allow him to stay on a visitor visa (valid until November 2022) at least until September 2022, when my study permit expires? Because I heard that over a visitor visa, a maximum single stay of 6 months is allowed.

You will need to ask for the expiry date to match the expiry date of your study permit and you should have proof of you study permit. Normally visitors are only granted a maximum of a 6 month stay but your child is not really a visitor but a child of an international student so you can request that the expiry date match the expiry date of your international study permit. If your child is only granted a 6 month stay you will have to extend right before the 6 month expires.

In case they won't stamp my son's passport at POE, does it mean they approve his stay for longer than 6 months? And in case I need to apply extension then should I go with visitor record or study permit for my son?

Yes no stamp is 6 months. You only need to extend his visitor status. No need for a study permit at his age.
