I got request for medical reassessment which was completed today. The xray and reassessment form were completed by physician. However I did not carry Page 5 and 6 of below
Completed MEDICAL REPORT CHEST X-RAY REQUISITION AND REPORT form (Page 5 and 6 of IMM5419) (in PDF)
Are we supposed to take print of this and carry? Or would the panel physician fill this form and mail it along with xray?
