Since I am a smoker, will there be any problems with my medical report? And does it have any effect on the visa decision? (Sorry silly question but just a bit paranoid since I am going in for my medical next week)

No, it does not matter.

In case Doctor says something, just reply that,

“If I cannot smoke in heaven, then I shall not go there….forget about Canada.”
