You should send the police certificate alongwith your paper file.
Upfront medical also you can send but it is not advisable because sometimes the application processing time exceeds 1 year then your medical will expire and you might have to submit new medicals upon reuest. So ita always better to wait for the medical reuest from your local visa office.
