Hello,

I applied citizenship for my bro-in-law and when he and my husband went to show all the documents, they gave a medical opinion form to take it to the doctor. The doctor made some mistakes and do any of you know where I can download the form and print. Its a request for medical opinion form, at the bottom it says CIT 0015 (09-2013) E. Thank you.

I have never heard of this... I searched online but there is nothing even online about it...

Can you please explain what this form contains? There are no medical questionnaire with citizenship...
Can you possibly upload a redacted version of the form or explain what kind of questions are on the form?

Did your brother-in-law apply under a special needs context? Either way, you cannot download this form - it's provided by the CIC Level-2 citizenship officer at the interview. You could go to the local office to solicit one, or have the physician correct the current form and initial each correction.

I am new to this forum and having hard time figuring out the whole reply thing.

Yes my brother in law has learning disabilty so we had to apply on that. We initially send Doctor's letter with the application as we can't prove language requirement. I was looking online for the form and I found the link about the officer 2 making decision. Thank you for replying.
