Medical, no IMM1017?

My spouse completed her medical here in Toronto a few weeks ago,

The paperwork we got from the doctor was front sheet stating that he is a DMP for immigration assessments and confirming that my spouse had her medical on September 17th. With a signature.

The second page is an e-medical "tracking sheet" with my spouses identification information.

There is no third IMM1017 sheet that is required according to the document check list.

What document are we suppose to include with our common-law application?

The physician should have provided you with an information sheet. On that sheet will be the applicant's picture, name, date of birth etc, and what tests were carried out.

The physician would have submitted all of that information electronically using a system called eMedical. That information will have been submitted to CIC.

In your application package just include a copy of the information sheet your physician gave you.

So does that mean the IMM1017 listing in the checklist is incorrect? Also should I include the page with the doctor's signature?

No the checklist is correct, just provide a copy of the document(s) the doctor gave you.

The checklist states

Proof of medical examination for the sponsored person and each of his or her dependants (Copy 1 of Medical Report: Client Biodata and Summary (IMM 1017), signed and provided by the panel physician)
