But unfortunately the document name for medical insurance didn't appear.
So which question I should answer to show medical insurance document in checklist?

Show us a pic of the checklist. I'm looking at my mom in law's, and it clearly says proof of medical insurance is required.

Proof of medical insurance is mandatory for the super visa application. You must include it when you apply.

Do check the visa office instructions and Document Checklist for your mom's home country here: http://www.cic.gc.ca/english/information/applications/visa.asp

One common mistake people make too, is they end up applying for the regular visitor visa instead of the supervisa, and regular TRVs don't require insurance.

Double check to make sure you really are applying for the supervisa. I remember I couldn't see anywhere on the app form that said "Super visa", and I had to change one of my questionaire answers regarding my wife's relationship to her mom, and it produced a different visitor application that was clearly for parents.
