Medical forms not recd after mail from immigration authorities?

I have recd mail from immigration to pay Right To PR fees and they will send
Medical forms in 48 hours but its more than 2 months no form have been received?
Although we have paid reqd fees

Reple back to their email that youare yet to recieve the medial forms. You may also check at CIC designated clinics to see if your name is on the e-list of those eligible for medicals. If yes, proceed with your medicals.
