Medical exams are only for people that will be sponsored in the present application, or might be sponsored in the future. So if you are the principal applicant being sponsored by your wife, medicals exams are for you, and your children if they are not canadian already, and children in the US that are not coming with you now, but you might want to sponsor them later ( if they are not included as "non accompanying dependents and medically examined now, you will never be able to sponsor them).
