I was nominated under SINP and applied for PR last September. I just got a medical request from ottawa and am little confused. They attacehd three documents including Medical Report Section A, Instructions to Doctors, and Medical Instructions, but I think I need Form IMM 1017 & Form IMM 5419. Those two forms can be printed out from CIC website ? or I go to medical examination with only Medical Report Section A, Instructions to Doctors, and Medical Instructions ? If anybody did medical examination in Regina SK, does it take a whole day to complete the examination ? I will call and make an appointment for medical examinaiton tomorrow.
 
Usually you just go to the doctor's with Section A and your passport. They have all the rest of the sections. But when I did my ME I printed out all the forms when I got the the nurse laughed and told me "we have them haha you are well prepared".

Just google and you'll find those forms.
 
Thanks jrjayl for the advice. I just knew the medical form A is Form IMM 1017. How much is the medical exam ? How long does it take ? I may have to skip a work day for this.
 
Make an appointment with the clinic and go there at exact time. The ME is $200 and takes around 30mins-1hour. I don't think you need a whole day. But half day is necessary.

Actually different doctors charge different rate. One doctor I called charged more than $200 and the other charged me around $150 ... so I suppose if you live in a bigger city, it's best to call around to get the lowest rate

When you make an appointment with the doctor's office, ASK SPECIFICALLY what they need. Some doctors office need a passport sized photo to be glued to your Medical form.

It's not going to take half a day.. an extended lunch ... depending on the distance to your doctor might be enough.
 
I got an appointment for medical exam next week. Sounds like it takes about 2 hours, and it costs $285 cash. I live in Regina SK, so there is only one place. Hope my insurance covers my medical exam, otherwise I will be mad about paying $285 for it.
 
Your insurance definitely won't cover it - you have to pay the fee yourself
