medical examination

I'm applying for Permanent Residence through common-law.
In November (2011) I had the medical examination required from the visa office when I received a 6 months extension of my tourist visa.
I would like to know if I have to take a new medical examination now that I'm submitting the paperwork for permanent residence...

Not necessary. The medical is good for a year. If during PR processing your medical expires, the visa office will inform you if you need to re-do it.

But it says i need appendix C filled out when i submit my application..... does that mean i would need to take this to the doctor's office to get them to fill it out where the examination was originally completed

Did you go to have your exam with a Designated Medical Practitioner (DMP)? If so, they should have attached photos and stamped it, sent a copy from their office, and given you a copy to submit with your application. I don't recall now where it was the actual Appendix C, or if they give you another receipt with the photo and stamp. Maybe someone else can clarify.

I believe you are just required to submit proof of medical, when you go get your medical done, keep all receipts, they are your proof. The doctor submits his part directly to immigration once all tests have been completed and the results are in. At least this is the way we did it when we applied inland...the whole Appendix C is so grey, I had to call 2 different CIC reps to make sure I was getting the correct information...

Yes, I think you need to do another medical examination. Appendix C is a mandatory document in the application and I have seen people getting their applications returned if they dont attach Appendix C. So I think its better to give another medical examination.
