I'm new to this forum. I was just wondering if anyone know how long before VO or CIC send a medical request if upfront wasn't done.

I don't think anyone can answer that question without more details. Did you apply inland or outland? where is your visa office? how long does it take to process PRs from that office? etc. It all depends on when an immigration officer takes a look at your file and realizes that there are no medicals on record.
