medical checks for my children

could someone please unravel my confusion, I am applying inland for sponsorship but my 3 children will not be moving or occupying me in canada, I do understand that they will still need to undergo medical checks in australia but how do we go about it are there any forms there dad needs to take with him I know what doctor to take them to in australia and if there are forms where do I access them from, and also do I need to include my 19 year old daughter as one of my children on my application as she is in a defacto relationship with her fiance and they have a child together so will she need to under go a medical aswell

1. The DMP has the medical forms for family class applicants. Read the instructions as to how many photos and they will require the passport. Tell the DMP the results should be sent to Ottawa not to Singapore.
2. If your 19 year old is in a common law relationship, then she would not be required to have a medical.

Im sorry but you have confused me what is the DMP?, and what do you mean they need photos and passport? I will be taking my medical in canada but my children will be taking there's in Australia so I just want to know what forms my ex husband needs so he can take the boys for there medicals, sorry but you have really confused me now

1. DMP Designated Medical Practitioner is the Dr. where you have the medical see: http://www.cic.gc.ca/dmp-md/medicalinfo.aspx?CountryID=2035&CountryName=Australia it has to be one of these.
2. You or the ex has to complete the following form http://www.cic.gc.ca/english/pdf/kits/guides/3906e.pdf for each child to take to the DMP with their passports and 2 photos. The DMP once the medical is complete will give your ex the receipt copy of the medical (IMM 1017) which you enclose with your application.

You really should re-read the instructions.

Ok this is the part that has got me worried my children do not have passports and I am not paying for them to get passport because it is not necessary as they will not be leaving the country as their father will not let them, he is more than happy to take them for the medicals but we will not fork out money for passports that won't get used, now what?

They must have some identity documents, birth certificate, whatever.
