Medical certificate IMM 5695

I got my medical...and in the instructions they mentioned about Interim Federal Health Certificate (IMM 5695) ... For individuals eligible for coverage under the interim Federal Health program...
I am confused..what is this..??!! Do i need to submit it...??!! I cant find it on CIC website... please help me...if anyone knows. ...

Why dont you just get to the hospital first?

Hello... actually hospital is not my city...so i have to travel minimum 5 hrs to reach the hospital . So i thought it ia better to confirm before going to the hospital....

Please I need some help with this as well, application type is IFHP and I have 57 days to submit is this the medical request anyone please jelpand how do I go about doing it
