I got "Marriage_Questionnaire.pdf" form along with MR/PPR

Following is what it says

- Attached application form "marriage questionnaire" to be completed by you and your
spouse and returned to this office. yourself and Spouse/Common-law partner. This must
be received at this office by:xx-xx-xxxx

Question is : Husband and wife both has to separately filled up this form or just the principal Applicant? What do you think?

Please let me know who has been through this situation before.

Are you talking about the 'Statutory Declaration of Common-law Partner' form? ??? Unless it is different from the forms on the CIC website. You can determine this by studying the form and it's wording. I am a bit jealous though, an Oct 2011 with MR. I guess you are from NDVO :'( ;D

this form is for the applicants who got married recently and Husband and wife both has to separately fill up this form.
