Mandatory military service: what documents will they ask for post-ITA?

Hi all,

I'm about to enter the EE pool and want to be prepared for anything they might ask post-ITA. More than a decade ago, I did mandatory military service and was later discharged on medical grounds. I don't have any discharge documents anymore, just my very brief service record (not in English or French). It's literally half a page with three lines of abbreviations and some signatures.

I read that people were asked to fill in this form: https://www.canada.ca/content/dam/ircc/migration/ircc/english/pdf/kits/forms/imm5546e.pdf
Is this the current form for EE PR?

What did you do if you couldn't find out/remember the names of all those people (supervising officer, commanders)?

Did they ask for more documentation apart from the above form?

I want to add all the information in my LOE from the start.

Thanks! I found the thread before, but it's quite old and there isn't much additional info. I will check YouTube though.
