"Mailing instructions" question, please

Hi Everyone,

I'd like to request one more help with a stupid question (sorry about that) regarding "Mailing instructions" on the FSW instruction guide. The Mailing instructions says that we need to add "NOC Category or Arranged Employment Category" and "Primary NOC 2011 Code: XXXX" above the Centralized Intake Office address in our application package.
I would like to confim whether this "NOC Category" are those 3 categories below or some category from the 2011 NOC such as "Natural and applied sciences and related occupations"

Category 1 - at least one year of continuous full-time paid work experience
Category 2 - permanent job offer from a Canadian employer.
Category 3 - student currently enrolled in a doctoral (PhD)

Could someone, please, confirm what is the correct category should be put together with the address of the Intake Office?

I am sending via FedEx. The FedEx package has a plastic with sheet of paper inside that contains the destination address, sender address, ID number... however this FedEx plastic takes the whole front of the box. Where did you guys put that information requested on the mailing instructions (Citizenship and Immigration Canada / Federal Skilled Worker/ NOC Category / NOC code...) ??
Did you print a sheet with this information as requested in the instructions and put in the back of the FedEx box ? (because we cannot put it above the FedEX sheet that stays inside that plastic)

How did you guys do that?
