Hello. My aunt is a canadian citizen and my grandfather (her father) is still alive but is 89 years old already. We believe that we are won’t be able to sponsor him since he is of very old age. Would I be eligible to apply for lonely canadian program and just state that he is very old and he is very sick?

Only option is for your aunt to sponsor him through the PGP program. She will need to meet the income requirements to qualify and then of course wait to enter the pool and be selected. He will need to pass a medical to be approved.
Just to add to my comment above, you could also sponsor him through the PGP program. You'll need to meet the income requirements and be selected out of the pool. Your grandfather will also need to pass the medical.
You are not elegiable to sponsor him under "lonely Canadian" but you could try to submit interest to sponsor as grandparent under PGP. You will need to wait for the submission to open up again (which hasn't been open since 2020) and be picked by the lottery system.
In either case (lonely Canadian/PGP), the applicant needs to pass medical exam. So it will be a problem if he is very sick already.
