LMO question and TWP question?

All,

My wife got a job in Canada and the employer wants to file LMO. I have a few questions.

1. She is British but lives in USA now, should she give them her UK address?
2. Can you use uk address but apply for temporary work visa in the USA?
3. For TWP, is there an interview and do you have to do medicals ?
4. What is the max years you can get for twp?

Depending on your intended occupation while in Canada, certain temporary foreign workers are required to undergo a medical examination. The following list provides examples of such occupations. This list is not all-inclusive.

Occupations that bring you into close contact with people, namely:
workers in the health sciences field
clinical laboratory workers
patient attendants in nursing and geriatric homes
medical students admitted to Canada to attend university
medical electives and physicians on short-term locums
teachers of primary or secondary schools or other teachers of small children
domestics
workers who give in-home care to children, the elderly and the disabled
day nursery employees
Agricultural workers from one of the countries listed here.
4. 4 years.
5. If she gets an approved LMO and doesn't require a medical, she can get her work permit at the Border.

I have been on a LMO from last 8 months but I have also applied for my PR under Canadian Experience Class last month. Am i eliginle to get an open work permit once I get my file no. As right now under a work permit with the company who sponsored me. Just want an open work permit. Pls advice.
