List of any visits made to Canada - ONIP Question

Hi guys,

When filling ONIP form, I have one question about list of any visits made to Canada. My case: I came in Canada in 2011 to study and graduated in 2014. I started working in 2014-now. I took vacation to my home country in 2013 and 2015. Should I state as below (I included all my visit to Canada, even for vacation and when my status change):
Reason for Visit Province Length of Stay Year of Visit
Work ON 4 months 2015
Work ON 1.2 years 2014
Study ON 6 months 2013
Study ON 2.5 months 2011

Do you guys have any recommendation?

Instead of the columns 'Length of Stay' & 'Year of Visit' you can use 'From Date' and 'To Date'.
