Letter of admissibility on my medical examination

Hello!
I would go to Canada for medical electives(clerkship) for a month Jan, 2017.
I have a problem with the visa.
I am South Korean who do not need any visa for staying less than 6 months in Canada.
Instead of the visa, I needed an eTA. so, I got it.
But the problem is that my purpose going to Canada (medical electives) is an exemption of medical examination waiver.
Though eTA is enough for the visa, I still need to check my medical status and prove this on the border of Canada.
That's why I did the upfront medical examination on Aug.

But I do not have any application forms to apply the letter of admissibility based on my medical examination.
I just sent an email to the Manila visa office two weeks ago. However, there is no answer yet.
I am worried not to get the letter soon, since I have to submit to the university I would go in Canada.
Anyone has the answer or similar experience with this?
