hello every one

i have added my mother's surname instead of my father surname .

i am a student on work permit .
recently i have changed my name . i got a certificate of legal name change from ministry of Ontario .......now i want to changed my new name on every where like my passport , on rest things its done changed on driving , sin number , bank cards , college degree

i want to know the procedure to changed on passport ????????????????????????
how long its take to be changed becz i will soon move to other province
documents required

1. Only Govt. of India can change your name on passport.
Fill up http://passport.gov.in/cpv/ANNEXUREE_change.htm and apply to Indian embassy or local passport office, India.
