LC2 new barcoded form NBI not included in the document checklist????

PLEASE HELP!!!!!! sino po d2 ang nag submit na sa CEM ng new LC2 barcoded form especially 2010 LC1 applicant. without submitting NBI???

hi, ask ko lang po kung di mo isinama yung NBI sa LC2 kit to be mailed to CEM? LC1 po ba kayo or LC2. Kasi LC2 po ako. The letter from CEM i got aug 8, 2012 (pero the date on the letter was july 27, 2012). Do we have to include IMM 0008 Generic Application Form? Yung LC1 lang po ba ang mag fill up po nito then affix signature? Thanks
