Last Name on birth certificate

Please suggest me something , I am not having birth certificate. On my 10th, 10+2, Bachelor degree all having my First name and Middle name only, there is no last name on these. Then I apply for passport which needs to include last name. After that all my documents having last name.

Now I heard about it that applications are returning back due to no birth certificate and immigration is not accepting CGI toronto birth certificate.. So asked my parents to get birth certificate from india... But we are confuse that whether it should have my last name or not? As I used only first and middle name only on all certificates untill I made passport/... PLease suggest..

Get an Affidavit done evidencing your DOB (either by your parents in India or yourself here in Canada) and go exactly by the name as it appears in your passport.

When you say get an affidavit here in Canada? Do you have any specific format ..wording ..could you please share with me ..
