I am ready to submit all documents and pay the fee. However, I have few questions before I do that.

- I have more than $12,475 CAD in my bank account right now (as of today's exchange rate), however, my six month average balance is less than that. Is that fine? All the source of the funds is legit i.e. salary and I can prove that.
- Do I need to attach credit card statements as well from the same bank for last six months?
- I got B grade in medical exam and the comment left was "No significant history or abnormal findings". I suspect it was because of my appendicitis removal surgery I had 17 years ago. Will this be a problem?
- My employer is located in location A and I am located in another state in location B at client's site. So, my work history will have employer address which is location A right? If it helps, reference letter states that I am at location B and it also has employer's address which is location A.
- Is it fine to change NOC code after ITA? I feel my roles and responsibilities are more aligned towards NOC 2174. The NOC I entered pre-ITA was NOC 2173.
- I've been in US since July 30th 2013 (exactly 5 years ago today
) first on student visa and then on work visa. I didn't go back in those 5 years. In travel history I've entered USA as country of visit since July 30th 2013 which is ongoing and purpose of visit "Student visa". Should I put "Student/Work"?
