My partner got an email requesting a newly Sponsor Questionnaire (IMM5540) bearing an original signature and gave us until March 3rd to mail it in.

but they provided a link for fastest service through IRCC webform; my question is for UCI and application number do I put the UCI that came with the letter or my UCI from my visitors visa? and is the application number the number that's right underneath that in the letter they mailed her?
(I want to make sure I'm doing this right, this is the second time that we had an error with our application :-[)

if the letter is addressed to you partner, and your partner has got UCI. you should be using it

That is the old form for the sponsor to fill out, and your spouse is the sponsor, I assume? So put their UCI (the one on the form). You both get a UCI number.

We ended up using both of our UCIs just in case.
and emailed it the same day still waiting to hear from them :-[
super nervous about our application since we had 2 errors already.... :-[ :-[
