IQAS Proof of Name Change

Hello,

In my passport, my details are like below.

Surname: Smith
GivenName: John

My name in all my education documents is like below.

Smith John

So, the Surname is coming before GivenName.

For filling IQAS application, it is asking for two documents. One for "Proof of Name" and one for "Proof of Name Change".

Can I give my birth certificate for "Proof of Name" and Passport for "Proof of Name Change". Please help.
