Inland Police Certificate App help

Filling out the Police Certificate Application
Its for Jamaica and asks place of birth..then address. Would i put my husbands current address in Canada here or maybe use the person he is sending in jamaica to do it for him address?

Also ask for name and address of recipient Agency (embassy, consulate, immigration etc) what is this??

And says need certified copy of front page of passport??? Where do I certify?

Q3 - you take a photocopy of the page of the passport and the passport itself to a lawyer who compares them both to make sure the copy hasn't been Photoshoped, and then stamps it with his firm's official stamp and signs it as a certified copy.
