Yes...there have been cases where in CEC or other immigration streams, applications have been refused due to a low score in only one module.
By low score, however, it means a score lower than criteria specified on CIC website. CIC mentions minimum scores for each module as well the overall score that must be met in order to prove language abilities.

I am not sure what these minimum scores are but you could check online.

PS - You must meet the individual module minimum score requirement even if you meet the overall score requirement.

A refusal is guaranteed with this score.

You must score at least 6 in each module.
