Info on Payment Receipt

Hello,

My wife is applying for cirtizenship but I paid online through my credit card for the processing fee. In the name section of the receipt, it has auto-populated my name but the following fields are blank:

1. Client date of birth
2. Address
3. Telephone.

Do I put in my information or my wife's especially for Client DOB?

I have searched all over the place but can't find a clear answer to this so any and all insights are appreciated.

If you applying for your wife, the blank one should be her information and not yours
