Do we need medicals for new born baby while applying TRV ?

Its mentioned as Upfront medicals. If so how to take the exam?

GC key or new portal which is preferrable.

Parents are outside canada , so we are planning to apply TRV first and then PR coming to canada.

for TRV, do we need to medicals and if so, should we need to get it done in the physical panel location mentioned in cic site? Thanks
