Outside India. I am in USA. This form is downloaded from New York Consulate for Police Verification.

lampbreaker is right. The form is in raster image form at the bottom of the page of the link you provided.
I applied from within Canada and here, there exists a similar version called the 'Personal Particulars Form'!

I wish they provided an actual PDF instead of an image. The quality when imported to word is really bad.
