Important : Question regarding Application Form

Hello all,

Please give me some guideline for filling application form

For adult:
Q5: List of all names at the time of birth , before and after marriage etc:
could I print the same name what I have on my immigration documents?

For Minor:

Q14:
Parent Signature: Should both parents sign in a sign bix?
Child Signature : my daughter is 5 years old . should we leave this child signature empty ? or can we print some thing like NOT APLICABLE ?

In addition to that , there is issue in heading row of table in the application form , it is getting multiply 4 or 5 or more instead of original one. please see attached one . How to solve it ? even I tried to fill new form for multiple time but it is adjusting the space what I am thinking.

please guide me question wise then i will be so thankful for this act of kindness.

For Q1: the instructions says that if you had a different name before other than the one you’re applying with.

For Q2: Only the parent applying signature and no signature for kids below 14 years old.
