Immigration problem

Here is my record. Last correspondence i received Feb.18, 2016 is my problem. Regarding my husband already not included in my application but need to undergo immigration examination on refugee protection act and regulations need to submit another medical after the expiration of the 1st one, but how we gonna do without the request/instruction form cic for the doctor? Any ideas please...The letter so confusing.

Permanent Residence
1. We received your application for permanent residence on June 14, 2012.
2. We started processing your application on October 4, 2012.
3. We sent you correspondence on July 30, 2014. If you have not yet provided the information or the requested documents, please do so as soon as possible. You do not need a request from CIC to be given to a doctor to do your medicals.
