IMM5710E issues with a couple of questions

Hello, this is my first time writing on a forum so please forgive me if I make any mistakes.
I am in the process of sponsoring my wife for her permanent residency. I am Canadian and we both live here in Canada.
She is currently working in Canada full-time in her intended job.

I have arrived at the point in the process of filling out her IMM5710E (open work permit application) but I'm stuck at a couple of question that are a bit tricky for me to figure out, so some help would be much appreciated.
My issue is in two parts of the form, one is on page 3,(Detail of intended work in Canada), question 5 (duration of expected employment) and the other is on the same page but is under Employment, part 1. The duration of expected employment is tricky because her contract states no ending date of employment and the letter from her employer states that it is a permanent position, but the form requires a « from » and « to » date, what should we write for both situations? Page 3, question 5 wants to know when she intends to start working for her present employer, I really don't know what to write because she is already working for that employer and the form wont' allow you to write a date previous to the date you are writing the form. But it also wants to know a (To) date and her position is permanent its also what her contract states, so we don't what to write. Should I write the date that her work visa ends?

The other question is on the same page where it says employment and that requires that you write your employment history and again her current employer is on that list and were not sure what to write for the (to) date as she intends to keep working for the same employer.

Again any help would be great seeing that the CIC help line just keeps screaming at me and telling me that the information is in the guide instruction and that's absolutely not the case. Thanks again for your time.
