IMM5669E - unfinished degree, short input fields and mother's maiden name

Hi all
I have been requested to submit an IMM5669e form, being an FSW Outland applicant
I have some questions regarding filling it in:
1. I was doing a PhD for 3 years, but did not finish it - should I mention that in the section 7 - Education? I did not claim points for that, only mentioned that in the personal history when applying.
2. The name of the institution and specialization does not fit in the Education section. I can put an abbreviation and then I can attach an additional sheet with a table that contains the full name of the institution and specialization. Should I do that or what should I do in such a case?
3. My mother took my father's last name - should I mention her maiden name or current one is enough?

1) Mention it in personal history and in the activity field, mention that you didn't finish ( explain in additional page that you did not finish it if no space available).

2) Yes.

3) It specifically says 'family name at birth'. So, just give that.
